This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our pledge

Trauma Healing Support (“the practice”) is committed to protecting the privacy of your health information. We are required by law to maintain the privacy of protected health information, to provide you with this notice of our legal duties and privacy practices, and to notify you following a breach of unsecured protected health information.

How we may use and disclose your information

  • For treatment. To provide, coordinate, or manage your care — for example, consulting with another of your providers, only with your knowledge and, where required, your written consent.
  • For payment. This practice is private-pay. We provide superbills at your request for you to submit to your insurer; we do not disclose information to insurers directly without your authorization, except as required by law.
  • For health care operations. For quality, supervision, and administrative activities that support your care. Clinical consultation and supervision are conducted using the minimum necessary information.
  • As required by law and for safety. Including when there is a serious threat to your health or safety or the safety of others, suspected abuse or neglect of a child, elder, or dependent adult, or a valid court order.
  • With your written authorization. Any other use or disclosure — including most uses of psychotherapy notes, and any disclosure for marketing — happens only with your written authorization, which you may revoke at any time.

Your rights

  • Inspect and copy your health record, with limited exceptions.
  • Amend your record if you believe it is incorrect or incomplete.
  • An accounting of certain disclosures of your information.
  • Request restrictions on certain uses and disclosures. If you pay out of pocket in full for a service, you may require that information about it not be disclosed to a health plan.
  • Confidential communications — request that we contact you in a specific way or at a specific location.
  • A paper copy of this notice, at any time.
  • Be notified if a breach occurs that may have compromised the privacy or security of your information.

Our duties

We are required by law to maintain the privacy and security of your protected health information, to let you know promptly if a breach occurs that may have compromised it, and to follow the terms of this notice. We reserve the right to change these practices; the revised notice will be posted on this page and available in the office.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with the practice or with the U.S. Department of Health and Human Services. You will not be penalized or retaliated against for filing a complaint.
